You can print out this form if you prefer to mail it with your check or money order. If you would like to pay via PayPal, just submit this order by clicking on the ORDER button at the bottom of the form and you will receive an email Request for Payment within 72 hours. U.S. orders are rolled and shipped in a tube; international orders are folded and mailed in an envelope.

PRICES: $5 for each sheet required to photocopy the map.

Number of sheets is given in brackets. For example [2] means 2 sheets at $5 each.
